This is a short lesson about Root Cause and managing risks in your ISO management system.
Why did it happen
Why did we not catch it earlier in our system (As applicable)
There may be multiple root causes (See points above)
Many methods, but the "5 Whys" is a very good standard process. Identifying the true root cause is absolutely critical to the entire process
Do not just restate the nonconformance
Root Cause should be a statement of fact
Initial versus mature system nonconformances
